Referring Physicians
Rheumatology Associates, PLLC, welcomes the privilege of working with you to take care of your patients with rheumatic diseases. We see patients only by referral from other health care practitioners. Our goal is to provide state of the art care in a compassionate environment, while providing the referring practitioners timely consultations with precise communication. We welcome your calls for curbside questions and our office will do our best to accommodate your patients in an efficient manner. Thank you for entrusting us with the care of your patients.
When Referring A Patient
Please complete the Patient referral form and fax along with Demographics, insurance information (including copy of insurance cards) all supporting information that pertains to the diagnosis. Keep in mind if the patient has seen a rheumatologist or dermatologist in the past, please include those records as well.
Once referral is received, we will contact your patient and notify you via fax regarding the patient’s appointment date and time.
