New Patients

Our Board-certified rheumatologist specialized in the treatment of systemic autoimmune condition, rheumatic disorders, MSK problems and osteoporosis. If you’re about to see us for the first time, you’re on the right path. Studies show the earlier you’re treated for your inflammatory arthritis the more likely you are to feel better sooner and stay active longer.

During your first visit with us you will have a comprehensive evaluation to determine the cause and origin of your symptoms. In many cases, a very extensive work up that include blood test, serologies and imaging studies is needed to complete the evaluation. A second visit is required to discuss the blood test results, imaging findings, differential diagnosis, and potential treatments.

Appointments

We value your time as a patient, and respectfully ask you to do the same with our providers and other patients in our practice. Being able to maintain an on-time daily schedule is a high priority at Rheumatology Associates, PLLC. As a courtesy to you we will deliver a reminder phone call to your number on record prior to your appointment with us, but ultimately the responsibility falls to you to keep track of the date and time of your appointment. Please understand that when you fail to provide adequate notice when canceling an appointment, fail to show up or arrive later than your scheduled time, it affects our ability to care for other patients. In order to protect our patients’ time and our office time we have employed the following policy:

  • We respectfully require that you provide no less than 24 hours’ notice for all cancellations, for established patients and 48 hours for new patient unless an unavoidable circumstance prohibits your arrival. There will be a $50.00 charge for any cancelled appointments and no shows for established patients and $200.00 charge for a new patient. This charge must be paid in full before another appointment can be scheduled with your provider.
  • If you arrive more than 10 minutes late for your scheduled appointment, you may have to be rescheduled
  • Repeated cancellations or no shows may be cause for your termination from our practice.
  • Please be sure to bring your insurance card, photo ID and copay with you to every appointment. We consider our time with you very important and ask that you turn off your cellphone when in the exam room.

 

Billing & Insurance

We participate with many medical insurance plans. We encourage all patients to read their insurance policy handbook to become familiar with their coverage and pre-certification requirements. If we are a participating provider on your plan, we will process your insurance for the services we provide. Your co-payment, coinsurance and deductible will be required at the time of service.

We require a current copy of your insurance card with each visit. If you do not have your card with you, we will ask that you pay for your visit at the time of service.

We accept cash, personal check, VISA, MasterCard, and Discover.

 

Pre-Authorization

If your insurance requires pre-authorization for procedures we perform within our office, please inform our billing staff so that we can obtain the appropriate pre-approvals prior to your appointment.

 

Prescription Refills

Call your pharmacy before contacting the office to request a refill once received from pharmacy requests for prescription refills will be filled within 48 hours. We will not prescribe narcotics of any type but will refer you to pain management if needed.

 

Medical Records

Your medical records are confidential. You must request the release of this information to any outside facility or agency. If additional copies are requested for other physician offices, government agencies, insurance carriers or your personal use, you will be charged for this service. Disability & FMLA

Rheumatology Associates, PLLC, charges an administrative fee for the completion of documents including Disability Forms, Family and Medical Leave Act (FMLA) forms, life insurance forms and return to work forms. Typically, your employer or insurance company requires these forms when a medical condition prevents you from reporting to work. A licensed medical professional must sign these documents verifying that the information is correct. If you need Rheumatology Associates, PLLC, to complete one of these forms, we will be happy to assist you. The fee for this service is $50.00. Please allow 2 weeks for forms to be completed.
