In preparation for your visit at Rheumatology Associates, PLLC, we’d like to have your health care history, insurance data and contact information at least two weeks prior to your initial visit. You can download the forms below. Once the forms are filled out, fax or mail them to our office.
Our forms are PDF documents that can be read with Adobe Reader. If you do not have Adobe Reader, you can download a free copy here.
We value your time as a patient, and respectfully ask you to do the same with our providers and other patients in our practice. Being able to maintain an on-time daily schedule is a high priority at Rheumatology Associates, PLLC. As a courtesy to you we will deliver a reminder phone call to your number on record prior to your appointment with us, but ultimately the responsibility falls to you to keep track of the date and time of your appointment. Please understand that when you fail to provide adequate notice when canceling an appointment, fail to show up or arrive later than your scheduled time, it affects our ability to care for other patients. In order to protect our patients' time and our office time we have employed the following policy:
We respectfully require that you provide no less than 24 hours' notice for all cancellations, unless an unavoidable circumstance prohibits your arrival (i.e. auto accident or medical emergency). There will be a $50.00 charge for any inappropriately cancelled appointments and no shows. This charge must be paid in full before another appointment can be scheduled with your provider.
If you arrive more than 15 minutes late for your scheduled appointment, you may have to be rescheduled.
Repeated cancellations or no shows may be cause for your termination from our practice.
Please be sure to bring your insurance card, photo ID and copay with you to every appointment. We consider our time with you very important and ask that you turn off your cellphone when in the exam room.
Billing & Insurance
We participate with many medical insurance plans. We encourage all patients to read their insurance policy handbook to become familiar with their coverage and pre-certification requirements. If we are a participating provider on your plan,
we will process your insurance for the services we provide. Your co-payment, coinsurance and deductible will be required at the time of service.
We require a current copy of your insurance card with each visit. If you do not have your card with you, we will ask that you pay for your visit at the time of service.
We accept cash, personal check, VISA, MasterCard, and Discover.
If your insurance requires pre-authorization for procedures we perform within our office, please inform our billing staff so that we can obtain the appropriate pre-approvals prior to your appointment.
Lab draws are available Monday through Friday during regular business hours. Many tests are run on a daily basis with results typically available to your physician within 48 to 72 hours. Other labs may take up to two weeks. The results
of the tests are sent to your physician for review. Your physician is alerted immediately by the lab if there is an abnormality that requires immediate attention.
Our office is equipped with on-site digital x-ray that allows your physician to personally evaluate your digital x-rays toward a diagnosis and assist with treatment decisions in a timely fashion.
Requests for prescription refills are handled during regular office hours and will be filled within 48 hours. Always check with your pharmacy before contacting us as refills may still be available on your present prescription.
Your medical records are confidential. You must request the release of this information to any outside facility or agency. This policy allows us to protect your privacy. Please make us aware of the name and address of your local physician
who should receive a report of your office visits. If additional copies are requested for other physician offices, government agencies, insurance carriers or your personal use, you will be charged for this service.
